A) Imp Note : The following set of documents self attested by the Holder are needed along with attached forms provided in below link

• PAN Card Copy 
• Valid Passport copy
• Valid Visa copy
• Valid OCI Card / PIO Card Copy. (If Passport is other than Indian Passport)
• Valid Resident Card / Permit Copy
• HDFC Bank Cancelled Cheque

B) In case if Holder is not in India currently, then the above sets of documents need to be self attested by the Holder as well as attested by Anyone of the following

• Notary Public
• Any court of law 
• Magistrate 
• Judge
• Local Banker - Bank Seal; Name; Designation; Signature; Employee Code of the Official doing attestation
• Indian Embassy / Consulate General of the country

C) Income Proof will be required duly signed by the Holder. (Any One)

• Last 6 months Bank Statement
• ITR Acknowledgement Copy
• Last 3 months Salary Slip
• Latest Demat Account Holding Statement
